Dear Sir

 With reference to the subject mentioned above I want to withdraw Rs xxxxxxx.xx for medical treatment of my mother but I don't have a cheque book to withdraw such a huge amount. As a result I unable to withdraw money from my account. So Kindly allow me to withdraw the said amount of money form my ac xxxxxxxxxxxxx or issue me a or processed cheque book if available and also I want to apply for a personalised cheque book.
